Choices In Learning Charter School requires the appropriate paperwork before a student can be admitted.
Below you will find links to the necessary paperwork. Please click on the Admissions_Application link, print the form, fill it out, and bring it to the office with all the necessary documentation. The list of required documentation for admissions can be found by clicking the Checklist link. Forms must be returned to the office. Faxed or mailed copies can not be accepted.
Our school has a dress code requiring school uniforms. Click on the Admissions Commitment Contract link below, print, fill out, and return to the office. This form must be returned to the school office prior to the child being considered for enrollment.
